Instructions for CalRecycle forms

NOTE: We will send these forms to you through DocuSign. Please DO NOT submit these forms directly to CalRecycle.


Beverage Manufacturer/Distributor Registration Form

  • Please answer ALL questions on the form.
  • Your DBA and Legal Business Name have been filled in according to our records. Please update if necessary.
  • We have filled in "Date company (month/year) began sales or transfers of beverages in single use containers in California" with “01/2024”. Please leave this answer as is.
  • For Please check the “ALL” box when answering if you bottle/can your own beverages.
  • If you need more space for the "beverage suppliers, co-packers and/or bottlers" or "distributors", please email us and we will assist.

Agreement to Report and Pay Applicable Fees Form

  • To simplify your completion of this form, Zero Link Markets, Inc. (DBA: Vinoshipper) has pre-filled its required information.
  • We have automatically filled in “On Behalf Of” and "Entity Paid For" with your Legal Business Name. If you change your Legal Business Name on page 1, these fields will automatically update.
  • "BM and/or DS ID#” is an optional field. Unless you have already registered (see below), leave this blank. These IDs will be issued after the forms have been submitted.
  • For the signature, change the Full Name to your name, choose a signature style or customize it, and click sign.

Change form signer

  1. Open email from Docusign.
  2. Click "Review Documents".
  3. In the top right corner, click "Other Actions".
  4. Select "Assign to Someone Else".
  5. Enter your Business Name in the "New Signer's Name".
  6. Enter the person's email who should be signing this document.

We have already registered with California

  1. Email regulatory@vinoshipper.com telling us you have already filled in the registration form.
  2. We will send you an updated email with just the "Pay of Behalf of" document.

I have multiple Vinoshipper accounts. Do I need two forms?

  • If the Legal Business Name is the same for all accounts, no. Please fill in the DBA section with all necessary DBAs.

  • If the business are completely separate, you will need to fill in a form for each business.
